The endnotes in the PDF version are not all quite right in at least one
wikibook¹. Many are ok, but others seem wrong. For example, on page 2
(and many other places) many of the superscripts do not have a number,
but instead have empty square brackets [] On other pages the place for
the endnotes contains empty vertical space, such as on page 12.

Is there something I need to correct in the Wiki version for the
endnotes to turn out right?

It seems that it is the <ref></ref> citations that are not making it
into the end notes.
